If you have had a medical checkup at another medical institution, please submit a copy of the results of the checkup (please include your student number and name) to the Health Service Center. If you need a health checkup certificate issued by the Health Service Center, you need to see a doctor at the Health Service Center (*see below). If you do not need a health checkup certificate, you do not need to see a doctor at the Health Service Center.
※If you need a health checkup certificate issued by the Health Service Center, please bring a copy of the results of your examination for the current academic year to the Health Service Center and see a doctor (please check the website of the Health Service Center for the doctor’s office hours). A few days after seeing a doctor, the Health Service Center (Higashi-Hiroshima Office, Kasumi Branch Office, and Higashi-Senda Branch Office) can issue the certificate of health checkup. Please note that certificates cannot be issued from automatic certificate machines. In addition, a certificate cannot be prepared with the results of a medical examination taken before the previous academic year.
[Issuance of certificate]
If it includes a blood test, the Health Service Center cannot issue a certificate. Please consult another medical institution. In addition, if a medical institution is designated as “limited to national and public hospitals”, the one issued by the Health Service Center will not be effective. Please visit a national or public hospital.
Please read the certificate instructions carefully when applying. When applying, please bring not only the prescribed form of the health checkup certificate but also the application guidelines.
